The piture of the old guy being treated by my medic is about 75 years old (wearing the long white "man-dress"
Dish-dasha). He suffered a wounded foot from and alleged shrapnal from an IED. He didn't seek any treatment and for about 6+ months it developed into what the closeup shows.
